Donna went to be with our Lord on Sunday November 24, 2019 at Doctors Hospital, surrounded by her children.
Born 09/14/1952 in Columbus, Ohio to Ernest and Juanita (Adkins) Jack.
Beloved mother, grandmother, sister and friend. 1970 graduate of West Jefferson High School. Animal lover and avid Cincinnati sports fan. Enjoyed cooking, bird watching and her flower gardens. Donna loved helping others and displayed that by spending time as a reading tutor in West Jefferson schools and delivering Meals On Wheels in Madison County.
Preceded in death by her parents, husband Roy Collinsworth in 2019, and her brother Cledith "Butch" Gibbs.
She is survived by sisters: Shirley (Glen) Williamson and Pat (Randy) Long; and brother Ernest "Mike" (Nancy) Jack. Donna also leaves behind four devoted children: Lori (Joshua) Ropp, Sharla (Jeffrey) Denney, Brian Collinsworth, and Todd (Misty) Collinsworth; eight grandchildren, Elijah, Jonah, Olivia Ropp, Charlee and Jessica (Tyler Payne) Biddle, Arianna Denney, and Lakyn and Ronin Collinsworth; one great granddaughter, Rylan Payne; and a dear special friend Chuck Taylor.
At Donna's request there will be no public services. In lieu of flowers, family requests donations in Donna's memory to the charity of your choice.
