Jack Dale Rayburn, 70, of Columbus, passed away December 31, 2018.
Jack was born June 25, 1948 to William and Bertha (Weekly) Rayburn.
He was an avid Ohio State football and basketball fan. He enjoyed fishing, camping, and riding his motorcycle.
Jack will be deeply missed by his caregiver Yonda Young; children Cristie (Frank Aldridge) Plott, Kelly (Ron) Smith, Jackie (Jason) Cole and Amber (Brady) Seymour; 14 grandchildren; 15 great-grandchildren ; sisters Helen Rider, Sarah (Kenney) Bensley.
Jack was preceded in death by parents, 3 brothers and 2 sisters.
Visitation will be Saturday, January 5, 2019 from 2pm to 4pm at Newcomer NE Chapel 3047 E. Dublin Granville Rd., Columbus.
Memorial services will follow at 4pm.
In lieu of flowers, memorial contributions may be made to Mount Carmel Hospice 1144 Dublin Rd. Columbus, OH 43215.
