Jonathan "Andy" Hook, 35, of Columbus, passed away Monday, October 3, 2016 at Mt. Carmel West Hospital.
He was born October 16, 1980 in Columbus.
Andy was preceded in death by his father, Billy Hook.
He survived by his mother, Tina Wood; grandmother, Dorothy Wood; companion, Danieal Grover; daughter, Jayda Hook; son, Jonathan Hook Jr.; sister, Jennifer (Phillip) Cornell; brother, Justin Hook and many aunts, uncles, nieces, nephews and cousins.
Visitation will be Thursday, October 6, 2016 from 4:00-7:00 PM at Newcomer Funeral Home SW Chapel, 3393 Broadway, Grove City, with the funeral service beginning at 7:00 PM.
